Service agreement
A copy of company or limited partnership affidavit with objectives (issued not more than 6 months) and an official copy issued not more than 6 months
A copy of identity card of an authorized person. In case of a foreigner, use a copy of passport and work permit (for ordinary person only)
Value Added Tax Certificate (Phor.Phor.20) (or equivalent)
A copy of bank account or a copy of passbook with the account name
A copy of commercial registration or business registration, which must not be registered less than 6 months
